How do I prepare a Crop Production Record?

I have this assignment. It is to prepare a crop production record for either salad beans, cucumber or lettuce.
I know that a crop production record includes date of germination, number of seeds planted, weight of yield, time taken from germination to harvest but that’s about all that I know.
What else should include in my record?
What format should the record take? Point form? Table Form?
I’m puzzled as to what I should do.
